Code 912 after exiting a game

Is anyone else having this issue?

I've managed to fix my Code 912 issue I reported previously by turning on dynamic bitrate but now I have a different issue, much less serious though.

I can get into steamvr home and load into a game and play for hours, reliably.

But when I exit the game, I get stuck in an endless code 912 loop. 

This has never happened to me before but now in order to get back into SteamVR home I find I have to close the streaming app in the xr elite and then close and restart steamvr and the vive hub.
