Always update the firmware when they are available to have a better VR experience. During the update, do not plug in or out any devices, and follow the instructions strictly.

If you cannot update the firmware, try other USB ports and use USB 2.0 instead of USB 3.0. Uninstall and install SteamVR could also help.
